Getting Into the Subject

To provide context for the reader, it’s interesting to give a brief introduction about the anatomy and functions of the lips. It’s important to understand that they are not just a part of our facial anatomy that adorns the lower third of the face.

The mouth is the organ through which food is ingested and sounds are emitted for communication. It is located in the anterior part of the human head, is an oval cavity, lined with oral mucosa. It is bounded by the cheeks, the hard palate, and the floor of the mouth. It is enclosed by the soft palate and an anterior wall formed by the lips.

The mouth is characterized by the presence of lips or fleshy folds bordering the entrance, the teeth, and the tongue. The lips are one of the most sensitive tissues in the body. They lack protective cells, sweat glands, fat, melanin, and keratin. They are very vulnerable to the effects of the sun, lose moisture easily, and can dry out. They can be clear indicators of certain diseases affecting the body.

Lips are so important and we use them so routinely that we often don’t realize or become aware of the need to care for them. Thanks to them, we can kiss, speak, or whistle; these gestures are possible precisely because of their simple but highly developed muscle layers. Thus, while the lips are a tiny part of the face, they represent one of the most transcendental areas of our lives.

Anatomy and Physiology of the Lips

The lips designate the external part of the mouth. They are two structures (one upper and one lower) formed by skin and muscle, providing entrance to the oral cavity. Their anatomical structure consists mainly of a core of skeletal muscle covered by skin (epidermis, dermis, and hypodermis) and internally lined by a mucosa formed by a lining epithelium, a lamina propria, and a submucosa. They extend from the lower margin of the nose to the upper border of the chin.

The reddish area is called the vermilion, and owes its characteristic color to the abundance of blood capillaries beneath a layer of transparent epithelial tissue. It is a transition zone between keratinized skin and oral mucosa. Since there are no sebaceous or sweat glands, the lips must be moistened with saliva to prevent dehydration.

The upper lip comprises the vermilion — the lip proper — and the area between it, the base of the nose, and the nasolabial folds. The lower lip is similarly formed by the vermilion and the skin area bounded by the continuation of the nasolabial folds and, inferiorly, by the beginning of the chin at the mentolabial groove.

The mouth is formed by a series of muscles, including:

  1. Orbicularis oris
  2. Buccinator
  3. Levator labii superioris
  4. Depressor labii inferioris
  5. Levator labii superioris alaeque nasi
  6. Mentalis
  7. Risorius
  8. Levator anguli oris
  9. Depressor anguli oris
  10. Zygomaticus major
  11. Zygomaticus minor

Volumizing and hydrating lip treatment with HA

Another type of treatment would be carboxytherapy. In short, this technique improves tissue oxygenation and thus repair of the area. With a protocol adapted to the patient, depending on the condition of this anatomical area, a series of sessions would be recommended. We can recommend this treatment individually or in combination with others.

With carboxytherapy, we achieve improved local circulation, increased nutrition of cells in the treated area, increased hydration, and stimulation of collagen production — all key for tissue rejuvenation.

In some cases, we could use botulinum toxin to improve the perioral area and give the lips a better appearance, or for the treatment of gummy smile — a condition in which the patient presents a disproportion in their smile that translates to an excessive visible area of the upper gum.

And lastly, we could employ microdermabrasion, using a device colloquially called a dermapen, combined with vitamins and/or HA to achieve hydration and slight temporary volumization of the lips without the use of injectables.
